Winter sport physical packets and recertification forms are posted on the Swimming & Diving team website (below). If a student is not currently competing in a fall sport, the full physical packet must be completed. If a student is already in a fall sport, only the recertification form needs to be completed. Families have the option of having a physical completed by their personal doctor. If interested in attaining a sports physical on Tuesday November 12, from 8:30 - 10:30 a.m. at Bishop McDevitt High School (and /or participating in a free EKG Heart Screen opportunity), please click on the link below and sign up for a timeslot. Please choose one selection per person and fill out the appropriate paperwork linked below.
